Health Ambassador Booking

    1.1 - Please endeavour to pay invoices in full within two weeks of invoice date. 1.2 - Places will be allocated on a first come/first served basis 1.3 - We try to keep numbers low, to ensure we give Health Ambassadors as much support as possible. We will inform you if the course is full. If this is the case, you will be offered alternative dates, but will not be charged until a new date is agreed. 1.4 - Confirmation of a place on the course will be sent via email on receipt of this booking form 1.5 - Completion of this booking form secures your place on the course 1.6 - Cancellation before an invoice has been paid: If for any reason you need to cancel your booking, an administration charge of £50 will be invoiced, unless your place on the course can be filled. 1.7 - Refund when invoice has been paid: If for any reason you need to cancel your booking, the maximum refund we can offer is £250 (for resources only). 1.8 - Further details will be emailed to you prior to the start of the course 1.9 - You will need to wear clothing appropriate for physical activity 2.0 - Refreshments are provided, but we ask that you bring your own lunch. 2.1 - If you are unable to take part in any of the activities, please share with the Course Director at the beginning of the day. We ask that you only do what you can manage. 2.2 - We ask you to sign an SLA (service Level Agreement) at the training to agree to working with us for a year to develop your programme. 2.3 - We ask you to fill in a School Health Check before your training, we'll then get in touch with you for a follow up Health Check after a year. 2.4 Half way through your first year with us, we ask you to fill in a feedback form.
